These terms describe the website, payment, assessment, and intake process for ProAging Longevity Lab & Vitality Center.

Service Description
The ProAging Assessment is a paid assessment and intake process intended to help determine fit, priorities, and possible next steps for a longevity or performance program. Payment occurs first through Stripe; the detailed health assessment intake is completed only after a paid session is confirmed.
No Emergency Care
The website and assessment process do not replace emergency medical care, urgent care, or a primary care physician. Anyone experiencing a medical emergency should call emergency services or seek immediate medical care.
Clinical Relationship and Program Fit
Completing checkout or intake does not guarantee acceptance into a ProAging program and does not automatically establish a doctor-patient relationship unless separately confirmed through appropriate clinical onboarding, consents, and professional review.
Client Responsibilities
Clients are responsible for providing accurate, current, and complete information; keeping contact information up to date; using the website lawfully; and not submitting information for another person without proper authority.
Payment and Refunds
Payment is collected through Stripe before the detailed intake is submitted. The $750 ProAging Assessment fee is non-refundable unless otherwise required by applicable law or separately agreed in writing.

Health Information
Health-related intake information and uploaded records are governed by the Privacy Policy and Health Information Consent. ProAging is responsible for clinical services and protected-health-information decisions; Violetek provides the supporting platform and related services as ProAging's business associate where applicable.
Payment Processing
Stripe processes card payments. Violetek provides the payment-processing integration for the initial ProAging Assessment; it does not receive a continuing commission or revenue interest in any separately purchased ProAging program, membership, laboratory service, supplement, coaching, or other later offering.
